Epidemiologic studies have consistently associated cigarette
smoking and occupational exposures to certain industrial
chemicals with increased risk of bladder cancer. Coffee
consumption has also been associated with increased risk,
but the data are less consistent than those for smoking,
and it is unclear whether this relationship is causal. Urinary
tract infections have also been suggested to be a risk factor,
and it is possible that consumption of vegetables or certain
micronutrients present in these foods may be protective.
Data, from a hospital-based case-control study in Milan,
Italy, were used to calculate attributable risks for bladder
cancer. Four hundred thirty-one cases and 491 controls were
included in the study. The overall attRIbutable risk estimates
were 49% for cigarette smoking, 23% for coffee consumption,
16% for low intake of vegetables, 12% for history of cystitis,
and 4% for occupation. These five factors together explained
more than 70% of all bladder cancer cases in this population.
These findings suggest that much could be done to prevent
bladder cancer in this population by applying current knowledge
about risk factors. The most substantial decrease could
be accomplished by eliminating cigarette smoking. Additional
bladder cancer cases might be avoided through dietary modification,
interventions to reduce urinary tract infections, and prevention
of harmful occupational exposures.
