diet and pancreatic cancer.
Loss of weight is a significant problem in pancreatic cancer.
There are a number of nutritional supplements that can be
taken as drinks to try to ensure that the patient gets sufficient
nutrients eg Fortisip and Maxijul and these should be available
from your GP.
Fortisip includes protein and fat as well as many vitamins
and minerals whereas Maxijul is a carbohydrate supplement.

ProSure has recently become available in the UK. Unlike
other products it contains 1g of omega-3 fatty acid EPA(Eicosapentaenoic
Acid).
This may help combat weight loss due to the cancer itself
(cachexia) rather than just as a result of not eating enough,
see news item in the Pharmaceutical Journal and abstract
in the journal Gut and BBC news: Fish oils 'help cancer
patients'. It should be taken under medical supervision.
There is still dispute about whether it is more effective
than other dietary supplements. Prosure is designed as a
supplement and not to replace meals. Other sources of EPA
are omega-3 fish oil (although the amounts per capsule are
not guarenteed) and oily fish such as salmon, mackerel,
sardines and tuna (see the information on the PanCAN pages).

Pancreatic insufficiency, that is decreased output of the
pancreatic enzymes lipase (responsible for digestion of
fat), protease (responsible for digestion of proteins) and
amylase (responsible for digestion of carbohydate) , is
common in pancreatic cancer patients and may cause vague
abdominal discomfort, pain, abdominal distention, excessive
flatus (gas), belching, diarrhea, steatorrhea (fat indigestion)
and weight loss. Pancreatic enzyme replacement therapy should
be considered for these patients. A good article on pancreatic

It is obvious that pancreatic insufficiency is likely to
be a problem with patients who have undergone surgery to
have part of the pancreas removed. However most non-surgical
cancer patients will have a blocked or partially blocked
pancreatic duct and so are also likely to have some need
of enzyme replacement therapy.
Patients should discuss whether they should be taking nutritional
supplements, supplements containing EPA or replacement pancreatic
enzymes with their GP, palliative care team or consultant.
Some patients will have become newly diabetic in association
with their pancreatic cancer. This means that they are possibly
trying to cope with diabetes and weight loss at the same
time. They should discuss with their dietician or other
medical advisors how to adjust their diabetes control to
compensate for changes in diet and the addition of supplements.
Some products are not suitable for diabetics. These would
be products that just contain glucose eg. Maxijul drinks,
Polycal drinks or glucose polymer powders (Maxijul, Polycose,
Caloreen).
Other supplements can often be used for diabetic patients
under dietetic supervision.
They do contain glucose and may therefore increase blood
glucose levels and therefore it is necessary to look at
the current diet and advise on intake accordingly. For example
if patients are unable to eat a normal diet then the use
of supplements instead of ordinary food is often necessary.
Patients can be advised on which supplements and when to
take them (eg. at mealtimes, between mealtimes etc) and
may be advised to drink them more slowly than would perhaps
otherwise be done to spread the 'load' taken into the body
in one go.
