Antibiotics and Dietary Supplements Prevent Stomach Cancer
Scientists Fight Stomach Cancer with Antibiotics and Dietary

Antibiotics and dietary supplements can thwart precancerous
changes that may lead to stomach cancer, researchers report
in the Dec. 6 issue of the Journal of the National Cancer
Institute.
Previous studies have shown that the two main risk factors
for stomach cancer are infection with a type of bacterium
called Helicobacter pylori (H. pylori), and a diet low in
vitamin C and beta carotene, which the body converts to
vitamin A. There is also ample evidence that a diet including
fresh fruits and vegetables, which can provide those nutrients,
protects against stomach cancer.
With this in mind, the researchers studied 1,219 people
in the province of Narino, Columbia, in the Andes Mountains,
a region with stomach cancer rates among the highest in
the world. People there with stomach lining biopsies that
showed precancerous changes were invited to participate
in the study. They were then assigned to a control group
receiving either a placebo (no treatment) or to groups receiving
one of the following treatments:
• antibiotics alone
• beta carotene alone or together with antibiotics
• vitamin C alone or together with antibiotics
• both vitamin C and beta carotene together, with or without
antibiotics
Depending on the type of treatment and the type of precancerous
changes, each treatment increased the likelihood about three-
to eight-fold that their next stomach lining biopsy, taken
six years later, would be normal. The researchers were surprised
to find that combining two treatments was no more effective
than one treatment alone.
Based on their observations, the researchers are very encouraged
about the potential of these treatments to prevent stomach
cancer. "My research shows that if we use the [antibiotic]
treatment, the infection will decrease and so will the incidence
of stomach cancer," says Pelayo Correa, MD, Professor
of Pathology at the Louisiana State University Medical Center
and lead author of the study. The findings offer solid basis
for further research, says Correa, particularly in the area
of developing vaccines for various types of cancer.
Infection Common in Asia, South America
The H. pylori infection is common worldwide, and has been
linked to increased risk of peptic ulcers. The bacterium
is thought to spread by way of food or water contaminated
with fecal matter. In areas where stomach cancer rates are
high, H. pylori stomach infections are extremely common
and develop very early in life, often as a result of poor
sanitation and crowded living conditions. The microbes multiply
and dwell silently in the stomachs of most children by age
10, and they often remain for life. In other parts of the
world, H. pylori infections are slightly less common and
develop later in life.
Correa has studied the bacteria and its effect on the stomach,
tracing its evolution into cancer, which he says can take
30 years. The study''s findings, he adds, will help physicians
treat those predisposed to gastric cancers -- a group that
includes those with a family history of the disease, and
those with diets high in salt and low in fruits and vegetables.
"The treatments [used for precancerous changes in this
study] are not very expensive," he says.
Findings of Global Significance
Eric Jacobs, PhD, a Senior Epidemiologist with the American
Cancer Society (ACS), says the study''s findings are important
on a global level because of the especially high prevalence
of the H. pylori infection in rural communities throughout
the world.
"This is encouraging evidence that, in developing countries
with a high level of infection and with poor nutrition,
curing H. pylori with antibiotics or improving nutrition
can prevent many deaths from stomach cancer," Jacobs
says. "On a global level, this is a very important
issue."
Jacobs says that while we would not necessarily expect the
same effects from antibiotic treatment in the U.S., the
ongoing studies "will help confirm the effectiveness
of H. pylori eradication and nutritional supplementation
in high risk countries."
